The role 
This is a brand new position sitting within a new innovative service navigation model for young people aged 12-25 years old living within the Peel region who are experiencing mental health issues & biopsychosocial complexities impacting their care. 

You will play a significant role in supporting the safe, consistent & high-quality delivery of care coordination & service navigation to the clients engaged in the service. This is a non-clinical role where your expertise, knowledge & skills centred around youth mental health support, risk assessments & more will be utilised. You will also contribute to service establishment, practice frameworks & continuous improvement. 

Key selection criteria 
  • Relevant tertiary qualification in Psychology, Social Work, Nursing, Counselling or related. 
  • Full registration with relevant membership body (AHPRA, AASW, PCFA, ACA). 
  • 2+ years experience supporting young people & families experiencing mental health issues. 
  • Demonstrated experience of psychosocial assessments of clients & referral pathways through a mental health service.
  • Excellent written & verbal communication skills and a client-centred approach to care.
